Requesting Medical Records

To request vaccination or other medical records from Student Health Services, please follow the instructions below. Medical Records are strictly confidential and no information is released without written authorization from the student.

Please see OhioHealth’s Notice of Privacy Practices (PDF) for more information on how your medical information is maintained and disclosed. Medical records are kept for seven years after the last attendance at Ohio Wesleyan, and then are destroyed. Requesting most parts of your medical record requires a written authorization and a potential processing fee.

All requests require:

• Full Name (name while you attended OWU)
• Date of birth
• Contact information (ex: email and/or phone)
• Date of last attendance (ex: Fall 2015)
• Departure status (ex: Graduate, Withdraw, Transfer)
• Who you want the records sent to?
• What is the contact information of the person receiving the records?
• What information are you requesting to be sent?

If you require ONLY vaccination records:

• You may submit this request via email to health@owu.edu including all of the information stated above. Shipping/processing fee will apply if sent via mail.

If you are requesting other information from your Medical Record on file:

• Please fill out the Release of Confidential Information form and return it to Student Health Services with the above listed information to health@owu.edu or fax to 740-368-3166.
