- Academic Advisor Change Request (Self-Service online form - login required)
To request a change of advisor, choose the new advisor from the list then click Submit request. Please confirm with the prospective advisor before submitting this form. Students who wish to change their advisor but are uncertain about a new choice should reach out to the Office of Holistic Advising for options.

- Permission to Transfer Courses (download fillable PDF form)
This form is for students currently enrolled at Ohio Wesleyan University who wish to request pre-approval to transfer courses from another institution back to OWU. Submit this form prior to enrolling in courses at another institution. Create a free account on Transferology to view approved transfer credit equivalencies. - Graduation Application (etrieve online form - login required)
Submit your Graduation Application no later than October 1st of your Junior year. (If it is already later than October 1st of your Junior year, then submit it as soon as possible. Why not now? It only takes a few moments.)

OWU is committed to protecting your privacy and complying with the Federal Education Rights and Privacy Act (FERPA). Please use the "FERPA Information Release" link to allow us to share your information with specific individuals.
- Petition for Special Permission (download PDF form)
Complete this form to petition the Academic Status Committee for special permission. Please note the petition must have all required signatures before it will be reviewed. Petitions are reviewed most Mondays at 4:10 p.m. during the academic year when classes are in session.
- Application to Audit a Course (etrieve online form or download PDF form)
No academic records are maintained for audited courses, so you will not officially be registered for the course and will not have a transcript. Informal auditing is not permitted. Persons not enrolled at Ohio Wesleyan may audit courses at Ohio Wesleyan University (providing space is available and the instructor grants permission). The $60 course audit fee is waived for residents of Delaware County, Ohio who are 65 years of age or older. Participants are responsible for supplemental fees associated with certain courses (activity fees, fine arts studio fees, etc.).

Use this form to officially change your permanent address in our system. You may submit this form to the Office of the Registrar.
- Application for Return to OWU (download PDF form)
Former students not currently enrolled at OWU who wish to return should complete this form. Students currently taking a leave of absence must complete this form as well. Applications deadlines are as follows: Fall – July 1, Spring – November 15, and Summer – April 15.
