Ohio Wesleyan University is gradually shifting our forms into our new etrieve/Softdocs system. When you see the login screen below, do not enter anything in the Username and Password fields at the top. Instead, click on the OWU logo at the bottom and log in with your OWU credentials.

• Change of Schedule Request (etrieve online form)
  Use this form to request course adds, drops, and withdrawals. 
• Permission to Transfer Courses (fillable PDF form)
  This form is for students currently enrolled at Ohio Wesleyan University who wish to request pre-approval to transfer courses from another institution back to OWU. Submit this form prior to enrolling in courses at another institution. Create a free account on Transferology to view approved transfer credit equivalencies. (See https://owu.edu/transferinfo and How Do I Use Transferology? for details.)
• Transfer Credit Conversion Guide (PDF form)
  This form converts OWU units into semester or quarter hours. The form also converts semester or quarter hours to OWU's unit system.
• Graduation Application (etrieve online form)
  Submit your Graduation Application no later than October 1st of your Junior year. (If it is already later than October 1st of your Junior year, then submit it as soon as possible. Why not now? It only takes a few moments.)
• Major/Minor Request (self-service online form)
• FERPA Information Release (self-service online form)
  OWU is committed to protecting your privacy and complying with the Federal Education Rights and Privacy Act (FERPA). Please use the "FERPA Information Release" link to allow us to share your information with specific individuals.
• Petition for Special Permission (PDF form)
  Complete this form to petition the Academic Status Committee for special permission. Please note the petition must have all required signatures before it will be reviewed. Petitions are reviewed mosts Mondays at 4:10 p.m. during the academic year when classes are in session.
• Audit Application (PDF form)
  Complete this form to audit a course. Please note that you will not officially be registered for the course and will not have a transcript.
• Election of Credit/No Entry (PDF form)
  Complete this form is to take a course credit/no entry.
• Change of Address (PDF form)
  Use this form to officially change your permanent address in our system. You may submit this form to the Office of the Registrar.
• Application for Return to OWU (PDF form)
  Former students not currently enrolled at OWU who wish to return should complete this form. Students currently taking a leave of absence must complete this form as well. Applications deadlines are as follows: Fall – July 1, Spring – November 15, and Summer – April 15.
• Application for Departure (PDF form)
  Complete this form either to take a leave of absence or terminate enrollment at the University.
• Appeal of Academic Dismissal
  This form is only available to students dismissed during the current semester. If you were dismissed in a prior semester, please use the Application for Return to OWU (PDF).
