- Course Withdrawal Request / Change of Schedule – Use this form to request a course withdrawal prior to the last day to Withdraw from courses. Note: This form becomes available after the Add/Drop period has ended for the semester/term. Course withdrawals must be requested prior to the relevant withdrawal deadline (see Academic Calendar). After this deadline, students may petition for special permission for a late course withdrawal, but approval of such requests are rare and depend on documentation of significant external factors beyond the student's control which prevented them from withdrawing by the deadline. No requests for course withdrawals will be approved during the final four class days of a semester / term, during finals week, or beyond.

- Application for Departure – Complete this form either to request a temporary leave of absence or to terminate your enrollment at the University. Students requesting to depart may be asked to meet with Dr. Brad Pulcini, Dean of Student Services, for an exit interview before the request is approved. (Questions can be emailed to btpulcin@owu.edu.)

- Credit / No Entry – Complete this form is to request approval to take a course as Credit / No Entry. This option allows you to earn course credit without a standard letter grade impacting your GPA. You must elect this option (with your Faculty Advisor's approval) no later than the Thursday of the second week of classes.
- Graduation Application – Submit this form by the start of your next-to-last semester to formalize your intent to graduate and request degree conferral and to request participation in Commencement / Graduation activities. Please fill it out carefully to ensure your ceremony year, name spelling, and phonetic pronunciation are correct for your diploma and the Commencement / Graduation Ceremony. Note: Commencement is sometimes also referred to as a Graduation Ceremony, though your degree is only conferred once the Registrar verifies all requirements have been satisfied.
- Petition for Special Permission (PDF) – Complete this form to petition the Academic Status Committee for special permission. The petition must be accompanied by any relevant supporting documentation and all required signatures before it will be reviewed. Approval of late requests for drops or course withdrawals are rare and depend on documentation of significant external factors beyond the student's control which prevented them from withdrawing by the deadline. No requests for course withdrawals will be approved during the final four class days of a semester / term, during finals week, or beyond. [Petitions are reviewed most Mondays at 4:10 p.m. during the academic year when classes are in session.]
- Transfer Credit (PDF) – This form is for students currently enrolled at Ohio Wesleyan University who wish to request pre-approval to transfer courses from another institution back to OWU.
