Ohio Wesleyan University is gradually shifting legacy forms into the Softdocs/etrieve system. When you see the Softdocs login screen (see example below), enter your OWU email address.

You will then be redirected to an OWU Sign-in page at https://idp.owu.edu/ (see example below) to log in with your OWU credentials.

 
 
  • Academic Advisor Change Request (Self-Service online form - login required) [preview]
    To request a change of advisor, choose the new advisor from the list then click Submit request. Please confirm with the prospective advisor before submitting this form. Students who wish to change their advisor but are uncertain about a new choice should reach out to the Office of Holistic Advising for options.
  • Appeal of Academic Suspension (Self-Service online form - login required. For the username, enter the first portion of your OWU email address in all lowercase. For example, if your email is btbishop@owu.edu, just enter btbishop without the @owu.edu). This form is only available to students suspended for the upcoming semester during the appeal window (12/17/2025-1/6/2026). Click here for detailed instructions on writing your appeal (download PDF).
  • Application to Audit a Course (etrieve online form or download PDF form) [preview]
    No academic records are maintained for audited courses, so you will not officially be registered for the course and will not have a transcript. Informal auditing is not permitted. Persons not enrolled at Ohio Wesleyan may audit courses at Ohio Wesleyan University (providing space is available and the instructor grants permission). The $60 course audit fee is waived for residents of Delaware County, Ohio who are 65 years of age or older. The waiver of the audit fee does not cover special or supplemental course fees (including, but not limited to, activity and fine arts courses). All participants, including auditors, are responsible for paying these specific course fees).
  • Application for Departure (etrieve online form - login required) [preview]
    Complete this form either to request a temporary leave of absence or to terminate your enrollment at the University. Students requesting to depart may be asked to meet with Dr. Brad Pulcini, Dean of Student Services, for an exit interview before the request is approved. (Questions can be emailed to btpulcin@owu.edu.)
  • Application for Return (download PDF form)
    Former students not currently enrolled at OWU who wish to return should complete this form. Students currently taking a leave of absence must complete this form as well. Applications deadlines are as follows: Fall – July 1, Spring – November 15, and Summer – April 15.
  • Change of Address (download PDF form)
    Use this form to officially change your permanent address in our system. You may submit this form to the Office of the Registrar.
  • Change of Schedule (Add/Drop) (Self-Service online interface - login required)
    Use Self-Service to add or drop courses prior to the last day to Add/Drop courses.
  • Course Withdrawal Request (etrieve online form - login required) [preview]
    Use this form to request a course withdrawal prior to the last day to Withdraw from courses. Note: This form becomes available after the Add/Drop period has ended for the semester/term.

    Use this form to grant access to your information to specific individuals (for example, parents).

    Submit your Graduation Application no later than the start of your next-to-last semester.

    You can also use this form to update your anticipated graduation month and year.
  • Permission to Transfer Courses (download fillable PDF form)
    This form is for students currently enrolled at Ohio Wesleyan University who wish to request pre-approval to transfer courses from another institution back to OWU. Submit this form prior to enrolling in courses at another institution. Create a free account on Transferology to view approved transfer credit equivalencies.   • Petition for Special Permission (download PDF form)
    Complete this form to petition the Academic Status Committee for special permission. Please note the petition must have any relevant documentation and all required signatures before it will be reviewed. Petitions are reviewed most Mondays at 4:10 p.m. during the academic year when classes are in session.
