Dear Incoming Students & Parents,

On behalf of Ohio Wesleyan's Student Health Services, welcome to OWU! Please take a moment today to complete your Student Health Services requirements. For the safety of the campus, only students who have met the documentation requirements are permitted to begin classes.

The deadline for submitting your information is December 15, 2023. If you are missing documentation after the deadline, your student account will be placed on hold until the information is supplied.

What You Need to Do

  1. Activate your OWU email.

    Once you have paid your deposit with the University, you will receive your OWU email address. Activate your email, set your password, and proceed. **If you are not reading this as a digital form that you can click on, you can find all of these links through our website: owu.edu/health, then view the Incoming Student Checklist page for steps 2 & 3.
  2. Complete the Required Medical Form.

    Once your OWU email is set up, complete the online Required Medical Form to provide your medical information. This includes the mandatory Tuberculosis screening questions. Your medical history/ family history is important if we ever provide medical care.
    • Sign in with your OWU login information.
    • Your user name is your OWU email address without "@owu.edu". Your password is the same one you use for your OWU email.
  3. Send us your immunization record(s).

    Please review our Immunization Policy (PDF) and print our Immunization Record form (PDF), and have your healthcare provider complete and submit the form, OR send us an official record that has been printed by your physician or high school. Students will also be required to provide documentation of receiving their COVID-19 vaccination. Please submit your records using the contact info below. Students requesting an exemption from any vaccination requirement should submit the form included at the bottom of the Immunization Policy. The University will follow up with those students and provide additional information.
  1. Complete a TB test.

    The online Medical Form in step 2 will tell you whether or not you need a TB test.
    • If required, the TB test must be completed within 6 months of beginning classes at OWU.
    • If you are unable to complete the test at home, we can administer the test after you arrive on campus in the Student Health Center for a fee.
  2. Submit Consent for Care form.

    If you are under 18, a parent or legal guardian must sign the Consent for Care (PDF) and submit it to the Student Health Center contact below.
