University Immunization Policy
Incoming students must also submit to the Health Center proof of vaccines from a health care provider. The OWU Immunization Record Form (PDF) may be printed and taken to your medical provider to complete, or you may submit an official immunization record from your medical provider.
Athletic Physical Forms
All athletes are required to have a physical with an OWU physician prior to participation in sports each academic year. There is no charge for the physical if completed on designated OWU physical days. Additional information and physical forms are available at the OWU Athletic Training website.
