All incoming students must complete the Required Medical Form online prior to arriving at OWU. You won't be able to attend classes until the form and additional documentation have been completed, submitted, and processed by the Health Center staff. See our Incoming Student Health Checklist for full instructions and additional information.
All portions of the Required Medical Form are due June 4, 2018 for Fall 2018 incoming students.
Incoming students must also submit to the Student Health Center proof of vaccines from a health care provider. Our Immunization Record Form (PDF) may be printed and taken to your medical provider to complete, or you may submit an official immunization record from your medical provider.
All students under 18 upon arrival to OWU are required to have a parent/legal guardian complete the Consent for Care form.
All athletes are required to have a physical with an OWU physician prior to participation in sports each academic year. There is no charge for the physical if completed on designated OWU physical days. Additional information and physical forms are available at the OWU Athletic Training website.

This policy is for any student on controlled medications for conditions such as ADD (attention deficit disorder) or chronic pain. Students able to continue care from their home doctor while at school are highly encouraged to do so, but the Health Center can help those who are unable to continue this care. For our physicians to prescribe any controlled medication, this agreement and the provisions explained within must be completed prior to any prescription being given.
The policy requires a claim form to be completed for proper processing prior to any payments being made by the insurance carrier. (See Insurance Information page for further instruction.)
