These Google Forms are used to collect important information associated with the day-to-day logistics of the Symphonic Wind Ensemble.
Student Information Form
This document should be completed by all Symphonic Wind Ensemble students immediately at the beginning of the academic year. Students returning in the spring semester do not need to complete this document again in the same year.
Instrument Rental Form
This document should be completed in advance of any university instruments that a student checks out for the semester to use in an OWU ensemble. $50 per semester covers maintenance and cleaning fees. Any lost or damaged equipment is the responsibility of the student and a hold may be placed on OWU records and registration until outstanding fees are paid.
